Submit Sports Participation Form to the NCAA
In some cases, the NCAA requires the submission of a Sports Participation Form as part of the admission process.
The Sport Participation Form provides the NCAA with information about your sporting activities during a period defined by the NCAA. Usually, it is used to find out whether you participated in tournaments, competitions, team games, etc. during this defined period.

If you find this task in your NCAA account, please speak to consultant immediately.
Use this form if you are a DI or DII athlete.
Use
this form if you are a DIII athlete
Choose the correct form from above and enter in the table on page 2 of the form all tournaments, competitions or games you have participated in during the specified period.
- Under Team or Club Name you enter the name of your club if it is a team game. For events in which you participated as an individual, leave this column blank.
- In the second column you enter whether the event in question is part of a national team or something similar. In most cases you will enter No there.
- Column 3 asks for the name of the tournament, event or league.
- In the last column you enter the date of the event.
- Question 1 below the table refers to periods or seasons in which you have not participated in any tournaments, competitions or games. You only have to answer this question if you have competed one (or more) complete seasons after high school.
- Questions 2-4 are only relevant if you have completed any form of military or civil service. In such cases, you describe what you did in this regard.

Do not submit the form without having your consultant check it!

Do not copy in your consultant or any other people into the email.
- Send the file with the saved answers by email to acp-processing@ncaa.org. The subject of the email should include your name and NCAA ID number.
